Tetri’s Smile (“TETRI”) limits a visit request to your name, the phone number or email you choose to provide, contact preference, general request category, preferred date, language, and contact consent. The form does not ask for symptoms, diagnoses, medications, insurance identifiers, records, images, or payment information.

When online delivery is available, the request is sent to the practice desk. If delivery is unavailable, the form tells you to call instead. Technical request data may be processed transiently to prevent abuse; application logs do not include the submitted contact fields. Requests are not sold.

This marketing site is not a patient portal and should not be used for clinical records. If you become a patient, clinical information is handled through the practice’s approved clinical systems and privacy procedures.
